If there is inflammation inside eye and doctor directly performs dilated exam with indirect opthalmoscope. will the inflammation be picked up in that case? or its important to conduct slit lamp exam first for "cell and flare effect" to diagnose it?

Yes: In a complete ophthalmic exam of a patient's eyes, a slit lamp exam for anterior chamber inflammation and anterior vitreous inflammation is performed. In addition the dilated direct ophthalmoscope exam will also look for vitreous inflammation. Hope this helps.
