Baby Stool: Constipation in babies is actually not common. True constipation is seen as hard pellet like stool. If your baby is pooping less, this may be normal. Don't give any water or juice or any other fluids other than formula. Make sure you are mixing the formula with the appropriate ratio of water to powder and feed your 4 wk old 120-180ml every 4-6 hours.

Newborn healthy diet: Either breastmilk or formula can be an appropriate newborn diet. However, putting baby to breast every 2-3 hours can help stimulate milk production which besides providing antibodies, calories and the appropriate amount of water, is a very good way to cure the constipation.
