I was recently in er for a heart attack which was later diagnosed as a dissected artery vessel and now im out of er and i need a work release form to go back to work i dont have a primary physician and what to do help please?

Find a doc: This is a public information site where volunteer docs answer questions about health conditions. It does not offer scripts/notes/releases. Work releases are legal documents. Docs who provide them without evaluating the patient can lose or have their license restricted. You need to see a local physician for such needs.

Your doc: Your primary care doc or specialist, if you see one, would be best to issue such a document, usually ER makes a formal referral to primary care if you did not have one. It has to be a doc who has access to your medical documents and can do an exam in order to issue a legal release.
