Her pediatrician : It sounds like your daughter has severe performance related anxiety that she will need to have addressed going forward. Best would be to coordinate with her pediatrician or GP, who may be able to address and/or refer as needed. Based on history and exam required documentation may be also properly issued.

Agree with Dr. P: When seeing her doctor it would be prudent to discuss if medication may be needed to tide over the stressful periods. If need be she may need to see a psychiatrist for a more detailed assessment and intervention. Wish you and your daughter good health!
