Doctor's notes cannot cover retroactive symptoms not reported at the time they happened for legal purposes, an exam is usually needed and tight follow up as many employers ask also for a work release. Objective evidence such as temperature, heart or breathing rate, abnormal findings on exam are required to support documentation.
