Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Wish you good health!

Back pain : Upper back pain with pins and needles in the hands and feet brings up the possibility that you have a neck(cervical) spinal stenosis. Cold shovers are difficult to connect to this , It is possible that you have an infection that has infected your spinal cord? The timing of this is not mentioned, I would be highly concerned if this happened quickly and suddenly. See a doctor.
