Short answer: 2-3x/daharmful hard accretions, especially those under the gum liney for 2 full minutes by the clock w fluoridated t'paste on soft t'brush. Brushing gently but thoroughly will remove 65% of bacterial plaque. To remove the other 35%, the part that does the most damage, requires daily flossing and/or WaterPik. Start w a professional cleaning by your Dentist.
