I need to have a medical form signed and notarized for an adoption home study. there's a number of lab tests that need to be performed and the final form has be signed by a physician. i'm wondering if it is possible to do this through healthtap?

Sorry, No.: The physical evaluation for an Adoption Home Study must be done in person by an MD. This includes a full History, in person Physical and blood work. The signature and notarization must also be done in person and cannot be processed through an online telemedicine visit. Call your family doctor and ask for an appointment for this in person exam.
