I havent received another pertussis shot since my first in infancy in 1981, due to getting a fever and rash in response to it. how risky is it to get it now during pregnancy? what could happen to me or the baby? what are the odds?

Short answer SAFE.: it is recommended to receive the TDAP during the third trimester of every pregnancy. This imparts protection from pertussis (whooping cough) to the new baby, For your particular case, discuss with your doctor, likely they will proceed with the vaccine, after talking with you.

Some basics: The pertussis vaccine you were given 36+/- years ago was the old whole cell vaccine that routinely gave patients reactions. It stopped being used in the US in the mid 90's when the lab derived synthetic vaccine replaced it. That new formulation has nothing to do with the old one & has the fewest reactions of the many vaccines given today. It is safe for both you & baby.
