Valsalva: Have been very down like he’s lifting something heavy and see if the swelling increases. If so, he could have an inguinal hernia. Please let me know.

Dr for evaluation: While there are many causes of low back pain, a young person like this with back pain plus groin pain & swelling should be evaluated by Dr for possible hernia or infection or even a tumor. Basic lab tests & imaging may be needed
