Scratched by a resident in a nursing home, she said that the resident was digging in her brief/ private area right before she was scratched and now it has scabbed up. is it possible for my sister to have an infection or something because of this ?

Not likely: Standard institution based procedures should be followed otherwise for transmission of possible infectious diseases.while the risk for contamination in theses circumstances via relatively low, following protocol in place is best to cover all bases.
