Many possibilities: Esophageal pathology, specifically in the context of 15 years of treated GERD and along with the described throat symptoms comes first to mind on location, but gastric, pancreatic, musculoskeletal pathology will have to be ruled in or ruled out. Seeing your gastroenterologist, hopefully you have one, would be recommended as the next step.
