Few possibilities: Could be migraine, cluster head ache. A condition called temporal arteritis or giant cell arteritis which can affect a blood vessel in the temple region which supplies a branch to retina can cause severe pain and at this condition risk of vision getting affected is there. Consult your PCP immediately.
