If you go to the emergency room with significant pain, can they discharge you if the pain isn't under control?

Yes: Happens every day. ER’s are meant for patients with life threatening conditions that are stabilized and admitted or discharged based on their medical needs. If work up is not pointing towards a significant medical condition, patients may receive symptom control meds and be referred to follow up with their PCP or a specialist. Pain control is attempted most times, not always achieved.
