Arm pain: Since the arm pain has worsened and can't be explained by a urinary infection, I feel you should be reexamined. The severe pain could be caused by a primary inflammation of a nerve in the arm or if a joint(elbow,shoulder) from acute tendinitis which is worsened by motion of the involved joint. Associated sweats, nausea, shortness of breath, chest discomfort would suggest cardiac origin.
