TM perforation: Either your regular doctor or ENT specialist may be able to see perforated eardrum on exam. Audiology testing (hearing levels and tympanogram) is also useful to uncover changes in hearing and mobility of the eardrum. In mean time, keep ears dry until this condition is ruled out

Easily identified: Any 1st year medical student can identify a perforated eardrum once they have had their basic patient exam course. It can be difficult under some circumstances of active infection & in some infants but it requires no advanced training. Most PA's nurse practitioners or otologists would also recognize the finding.
