Doctor can evaluate: A muscle strain is when some fibers get stretched too far and/or some fibers tear. People call it a tear if too many fibers break, leading to some bleeding and swelling inside the muscle, resolving later with some healed scar tissue and some tightness in the area (less elastic). That said, evaluation and diagnosis are first needed, to be sure there is not something like a tumor, infection, etc...

Chronic pain: This isn't a characteristic of muscular strain. I suspect an impingement of a nerve root in your neck or upper spine from an intervertebral bone spur or bulging intervertebral disc. I recommend consulting a neurologist. An MRI would be most diagnostic of a disc problem and a plain xray of the cervical and dorsal spine could demonstrate arthritic spurs. Avoid straining the neck and upper back.
