Vasoconstriction: Nicotine causes coronary arteries to constrict and reduce blood supply to the heart and could be responsible for some of your symptoms. It would be good to quit using tobacco while you are ahead. https://www.ncbi.nlm.nih.gov/pubmed/8354792

Chest distress: I agree with Dr. H as to the probable cause of your chest distress especially if you inadvertently swallow tobacco juice. I assume you were given Toprol to prevent cardiac arrythmias which can occur with MVP. MVP can also cause atypical chest discomfort itself.
