My son who is 12 years old had an ingrown toenail on his big toe that has got infected. we went to a doctor who put him on augmenting anti biotic to kill the infection. this has not worked and his big toe is swelling.

Unfortunately, undertreated ingrown toenails, in this day and age, can have serious consequences. Numbing the toe, removing part of the nail and infected tissue as well as taking cultures is usually the way to go. Antibiotics can then be in accordance with the culture results.
