See dermatologist: see dermatologist for eval and possible tx when lesions are active. If constant for 2 yrs, then could be due to chronic sun exposure and sun damage, called actinic cheilitis. I'm betting that the lesions are intermittent and there is a prodrome with tingling and itching/burning/numbness preceding an attack...if so, prob is hsv which means that you might need a culture and specific blood tests
