Are the painful : veins in your arms? Or in your legs? Your description of a "hard lump" could be a superficial thrombo-phlebitis (superficial blood clot) or simply painful varicose veins. I don't know how the shaking arms fit into this. Sounds like you need to see you PCP regarding this.

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. Three years is a long time to put up with painful lumps, please see your primary care provider for an initial assessment and to ascertain if you need to see a specialist. Wish you good health!
