Whenever i straighten my arm i feel a lump in the vein where my arm folds. its a hard lump that hurts. its the same vein they used to hook me to an iv for 3 days almost a month ago. is this a clot? should i be concerned? it just started today?

This could be a firm clot of blood from trauma during vein puncture from the IV needle.. It's most likely not going to dissolve at this point. A vascular surgeon can examine and advise you best.
