Not at all: 4 mg is a very small dose (doses of 25-30 mg are common). There is no known interaction between CBD and azithromycin. Higher doses of CBD (at least 25 mg) may inhibit some cytochrome P450 enzymes, slowing metabolism of some drugs, but azithromycin is not metabolized by those enzymes.
