More information is needed. One enlarged tonsil could mean that there is an abscess, but that usually causes a fever and change in voice. Small white spots with a "negative" strep can be a tonsillith ( a particle stuck in the tonsil of no big concern) which can cause an enlarged tonsil sometimes. The gold standard for strep is a throat culture.
