It is VERY: unlikely that a hiatus hernia would be missed during an endoscopic exam unless it were very small and thus unlikely to be symptomatic! Larger hiatal hernias can not be "felt" in the upper abdomen as they lie UNDER the diaphragm and are beyond the reach of palpating hands!!! Hiatal hernias may or MAY NOT be accompanied by reflux of acid . The esophageal SPHINCTER contols that! Hope this helps Z
