Probably no 1 thing: You've waited for a while for this answer because there is no answer. If she was 90 years old like your self, had already been on blood thinners and required a thoracentesis then we know that she already had more than one condition that could kill her and having to stop blood thinners to do the procedure says they were backed into a corner and likely she was suffering. Let it go, it was her time.
