Lots of options: History would be important in terms of understanding what might be the cause. Two year olds pretty much need to be constantly monitored so someone should have that information. If no one was supervising, then that supports the need for this child to be seen and evaluated. No common pathology other than trauma comes to mind-and need for urgent care
