Old Toothbrushes: Good day; Old toothbrushes can be thoroughly cleaned with mild dish detergent and then sterilized in a mouthwash. However, as brushes age the ends of the bristles flair outwardly indicating retirement.

Not smart: Better to discard and get a new one every 3 months.

Replacing Toothbrush: Toothbrushes should be left out to dry after use. They also should be replaced approximately every three months or if you see the brushes severely worn. Moisture and darkness contributes to the growth of mold whether it's on food or a toothbrush. While you could try to disinfect it, I would merely throw it away.
