Few possibilities: This could be either dental or cheek related. It could be a bacterial infection within the tissues or a viral or bacterial infection in the parotid gland (mumps, bacterial parotitis). If he was never immunized for it mumps is possib le but not the only germ that does it. He needs to be examined.
