Probably not: Blood under the nail, that causes a bruise. They can take a long time to clear up. Sometimes the black spot does not go away until it grows out with the nail.

No problem: The reason for draining these is to relieve pain. The red-hot paper clip trick is easy and painless. In your case it would be cosmetic only.

Hard to say: If/when the hematoma covers the inner half near the origin of the nail it can injure the growth bed & led to loss of the nail. That is uncommon but does occur. Early drilling/release of the pressure can prevent that. For most, it will just stain the nail. It grows out slowly, about 1-2mm/mo, with the stain disappearing from healthy new nail.
