Either: A very high prolactin level is going to be a prolactinoma, and prolactinomas are much more common that posterior pituitary lesions. I'd be surprised if any radiologist could swear up and down it arise too far back. The real question is, "Do we begin medical treatment for a presumed prolactinoma, or do we go in and biopsy because of the off-chance of a zebra?" Sounds like an easy call.
