My tsh is 5.45, t4 free is 0.92 anti thyroglobulin abs is 16.0, anti- tpo is 299.1. do i need medication for thyroid? my tsh was 6.37 in june.

Likley: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, based in the TSH levels you thyroid gland is functioning less than normal and it would be prudent to take thyroid medication.
