See your doctor: Although is is unlikely to be a blood clot, it is a good idea to be examined. Definitely recommend that you try to quit smoking for your overall health - but, smoking at age 20 does not really increase your risk of blood clots while you use nuvaring. We start to worry about that usually when you are age 35 or older.
