In 2015 my uterus was abnormal, long 15.0cm x ap 9.0cm x tr 9.5cm position: enteverted, endometrium 16.0mm. it has enlarged even more, is this bad.

Endometrium: If the ultrasound exam truly measured the lining of the uterus to be "16 mm", in a case where enlargement of the uterus is already established, a biopsy of the lining is indicated. You have not mentioned your age, but this kind of actual endometrial thickening needs attention.
