Long and short of it: Chonic diseases last a long time and dont resolve well. Example - chonic back pain a patient will say "i have had back pain for the last 10 years" acute diseases are usually sudden onset and resolve. Example - acute back pain. "i lifted a weight an hurt my back" i hope this helps.
