Find the reason: This souldn't be happening. You may have a stone that harbors a biofilm of these bacteria -- antibiotics usually won't clear this. You may have an anatomic defect of some sort. It's time for a serious investigation, starting with some imaging studies. Best wishes.

Resistance?: Do a urinalysis with culture sensitivity test with the lab>this will show which anti-biotic to use.Mantain hydration and make sure to finish the couse of anti-biotics.A prolonged course might be recommended by your doctor.E-coli is a common offending organism. Also follow proper hygiene when wiping yourself,it is front to back, not back to front.Good luck.
