More tests: Joint swelling and an elevated ESR indicate that you have joint inflammation, not just joint pain. This makes it suspicious for seronegative RA (rheumatoid arthritis with negative RF, negative CCP) or a disease in the lupus family. The ANA is not as helpful as you might think as it is positive in many conditions. However, you need DNA, ENA, complement testing now for diagnosis. You also need Rx.
