I have had si pain on and off for 10 years. ana, hlab27, crp, esr all negative always. mri in 2019 showed no sacrolitis. rf fact 10, anti ccp 4. joint pain on and off but no visible swelling or stiffness in back. any ideas?

Have you seen a rheumatologist? If it is not rheumatologic, perhaps getting physical therapy would be helpful. It is difficult to diagnose a problem without doing a full evaluation, including history and physical exam. Based on the labs provided, it may not be due to a rheumatologic issue but only the rheumatologist can say for sure. Following the labs over time sometimes yields the diagnosis.
