Possible reflux: When a person strains to have a bowel movement, abdominal pressure increases and puts pressure on stomach contents, which could then be causing the relux of stomach contents through her nose and causing spasm of the vocal cords. Keeping her upright might help. Have her doctor view a video recording of her while this is recurring to confirm my suspicions.
