Ectopic atrial tachycardia... does this arrhythmia start and stop gradually? what if it is exertional related? can rate vary since it is with exercise if it is the automatic mechanism type?

Usually this: arrhythmia will have an abrupt initiation and termination rather than a gradual one. Sometimes it can be exertionally related but usually the rate is not variable. Long term monitoring should be able to differentiate the mechanism for you but if not, an Electrophysiologist should be able to help you with either invasive or non-invasive testing.
