Cough, fever: This may be a viral illness, but could be pneumonia presentation, with silent breath sounds but a + CXR. For that, I would bring him to an urgent care clinic to review his status and determine if a CXR would be helpful to rule this out. Fever 4 year old: symptoms consistent with the flu but you did not mention the duration of symptoms. Would recommend seeing your pediatrician
