Few things: Possibilities include pleurisy, underlying pneumonia, muscle injury from coughing, broken rib. A chest X-ray may give you the answer. Tell your family physician or Urgent Care the symptoms and they will examine him and determine the treatment and if an X-ray or other tests are needed.
