Probably nothing: Unfortunately this is all to common in kids as they explore life. I have pulled out foam, beads, paper wads, bugs, etc. These are hard to remove sometimes even when the kid will sit still for the effort. (most 4 yo won't) Many will need to be removed as outpatient surgical procedure by an ear nose & throat specialist with special tools.
