Second opinion: Suggest you go to another well respected dentist in your area for a second opinion. It should not take 4 years to diagnose cause of pain. Once the cause of the pain is determined you can be given appropriate treatment options. Nerve damage requiring Root Canal Therapy or crown or root fracture are most likely causes of your pain.
