Safest to be treated: Most infections "go away on their own", including most UTIs. However, they can cause an awful lot of damage before they go. The risk from antibiotics is tiny compared to the risk of not being treated. Be rational about this. Also, be sure it's another UTI: often similar symptoms for other infections, especially STDs.

Recurrent UTI: Requires evaluation to determine the etiology which could physiologic like improper wiping techniques or poor sexual hygiene which require education or anatomic which will require radiologic or invasive test. At 21 recurrent UTI should not be ignored or observed
