Back: From a surgical standpoint I would probably go with the neurosurgeon. However, from a recent article from British Medical Journal only 5% of people get lasting relief with surgery. Perhaps physical therapy and weight reduction is the answer.

If the disk is-: -symtomatic but there is no evidence of instability,there is no need for fusion. Unless there is complete loss of the disk space. If the pain is mostly in the leg, & less so in the back,diskectomy only,as there is more back pain than leg pain,fusion is to be considered.
