Hyponatremia: Or low sodium is always of concern and should always be investigated (specifically when that low). A thorough review of her meds, symptoms as well as a physical exam are recommended. Further work up should include blood/ urine tests and imaging as deemed appropriate. Have your grandma see her doc as soon as possible.

Hyponatremia. : This significant, persistent hyponatremia should be investigated. There are some BP medications that can cause hyponatremia. But this should be addressed.
