PSA: I would recommend prostate biopsy at this point. Waiting will not achieve anything. If there is cancer present, you need to know. Treatment vs. No treatment should be thoroughly discussed if the biopsy is positive.

It depends: It matters how this reading fits your prior psa values if any. If it is an isolated reading or even your first ever, then repeating the psa ink the next few months is a reasonable strategy. There is no evidence that antibiotics have a role here and can even be harmful. If your prostate is large by exam this can also be meaningful. There is little downside to confirming an elevated level first.
