In general, yes: A 4-inch round vaccine reaction is big enough to go back in to have the doctor evaluate to see if any treatment is needed. Doctors also want to keep track of which patients get big reactions with which vaccines, although having a localized injection-area reaction after one dose doesn't always lead to the same reaction after the next dose. The doctor can judge and decide.
