You need to: have that evaluated! ("Routine" x ray to begin with) Sagittal band injuries ("BOXER'S KNUCKLES") are common , TEARS are rare . I assume you have swelling (and that's what making the knuckle look flat) but a tear or (less likely) an underlying FRACTURE is possible. Hope this is helpful Dr Z
